THE Research Committee of King Edward Medical University has granted technical approval to various research projects of Basic Medical Sciences and Medical and Allied Health Sciences. KEMU’s Research Committee issued a notification on Thursday after formal approval from Vice Chancellor Prof Dr Asad Aslam Khan.

In its report to the Union health ministry, exclusively available with TOI, the panel has recommended for the increase of undergraduate medical seats from 41,569 to 66,000 and PG seats from 20,868 to 31,000.

It has proposed setting up of 30 new medical colleges with public financing in states that need them the most. It has recommended setting up of 132 Auxiliary Nursing Midwifery and 137 General Nursing Midwifery schools through public financing with a focus on under-serviced areas and starting paramedical education courses in 149 government medical colleges and paramedical institutions.

India produces 30,000 doctors, 18,000 specialists, 30,000 AYUSH doctors, 54,000 nurses, 15,000 ANMs and 36,000 pharmacists annually. “Yet, geographic and rural-urban imbalance exists in training and availability of human resource. Medical colleges are unevenly spread across the states with disparities in the quality of education. Only 193 districts of the total of 640 districts have a medical college – the remaining 447 districts don’t have medical colleges,” the report says.

The summary has been rejected on the basis of certain objections and legal complications pointed out by the Law Department in the light of the Pakistan Medical and Dental Council (PMDC) Ordinance 1962, the PMDC Regulations 2010 and the UHS Lahore Ordinance 2002.

These laws clearly declare that the provincial government has no legal authority to bring the private medical and dental institutions under a uniform policy.

According to the documents obtained by Dawn, the Punjab Law Department in its recommendations had rejected any role of the PMDC and the UHS in regulating the private medical and dental colleges through centralisation of their admission process.

The health department also endorsed the viewpoint and requested the chief minister to ‘drop’ the scheme of bringing the private medical and dental institutions under a new policy for admissions.

Health Secretary Muhammad Jehanzeb Khan confirmed the development, saying the idea of introducing new uniform admission policy was floated by some participants in a meeting chaired by the chief minister a couple of months back.

Later, the case was re-examined in several meetings, one of which was chaired by the chief secretary, he added.The matter of regulating the private medical institutions by the Punjab government has remained a hotly debated issue at formal and informal meetings, discussions and various forums.

The documents say: “Government in the Health Department has no lawful authority itself to monitor or control admission process in private medical and dental colleges; however, subject to the 1962 Ordinance and the Pakistan Medical and Dental Council Regulations, University of Health Sciences can exercise reasonable control on such institutions through the tools of affiliation and disaffiliation and the terms and conditions prescribed for grant of affiliation and continuance of affiliation. For the purpose, the UHS is empowered to promulgate appropriate statutory instruments.”

It further says: “The Provincial Admission Authority (PAA), constituted by the provincial government to finalise admissions to the public-sector medical and dental institutions, has no legal authority to finalise admissions to the private-sector medical colleges.

“Each such college has to admit students on merit strictly in accordance with regulatory prescriptions of PMDC, and subject to that, by the affiliating University,” the law department recommended.

Law experts further clarified that as the centralised admission for the private medical and dental institutions by a provincial authority was not envisaged under the existing regulatory framework, the role of the provincial government was confined to holding of Central Entrance Test (CET) for the eligible candidates and
to constitute PAA for purposes of admission to the public-sector medical and dental colleges.

The health and law officers also cited references of the 18th Amendment in the recommendations to convince the chief minister.

They said: “By virtue of the Constitution (Eighteen Amendment) Act 2010, the subjects, relating to ‘legal, medical and other professions’ and ‘standards in the institutions for higher educations research, scientific and technical institutions’, have been shifted from the Concurrent Legislative List (Science omitted) to Part-II of the Federal Legislative List.

“Although, the subjects now fall within the policy making purview of the Council of Common Interests, the legislative and executive authority wholly vests in the Federal Government in terms of Article 142 (a) read with Article 97 of the Constitution.”

They further said the provincial government could not exercise executive authority in respect of any of the said subjects except to the extent that the federal government empowered it through statutory means.

“Now when the minimum requirement for eligibility to appear in the CET is 60 per cent marks in the aggregate in FSc and when no one can be admitted to any public or private medical or dental college without taking the CET, any apprehension that private medical and dental colleges would admit poorly qualified students to mint money may not stand the test on a rational basis. So the PMDC and UHS have to exercise regulatory controls strictly within the spheres allocated to each,” the law department said.

In the light of the detailed examination of the legal background, the health and law departments had asked the chief minister to maintain the Admission Policy (Session 2011-12) which he (the CM) had already approved in pursuance of the recommendations submitted by a committee headed by Senior Adviser Sardar Zulfiqar Khosa in August last.

KARACHI, Oct 2: About 250 faculty members of the newly-established Shaheed Benazir Bhutto Medical College in Lyari resigned en masse on Saturday citing serious concerns about their security and alleging, among other things, “physical abuse and sexual harassment”, officials said.

They said the “extreme decision” was taken unanimously by the faculty staff of all the 13 departments — seven of basic sciences and six clinical — after most of them experienced almost the same kind of threats, mostly affecting female members, constituting 70 per cent of the faculty.

“The faculty members resigned their positions and explained the reasons for it in a letter to the college authority,” said Dr Anjum Rahman, the college’s vice principal.

“In the entire situation the female staff, over 70 per cent of our faculty, appeared the most vulnerable. They have cited several episodes, including threatening phone calls, a kind of break-in and several attempts, which are definitely unacceptable.”

She said the resigning members included senior professors and the registrar, who were witness to incidents that took place over a year. There was no security measure for the college. Not even a regular police post for that purpose, she added.

“There are definitely people outside the college who are involved in all this,” said Dr Rahman when asked about the people threatening faculty members.

“The female staff members are so scared, and the resignation reflects the level of their frustration and sense of insecurity.”

Officials associated with the college said though the security concerns and threats remained there, the situation turned ugly only a couple of weeks ago when some ‘miscreants’ entered one of the facilities of the college, where a few female members were busy in their regular job.

“Some of them used abusive language and when staff members reacted, they attacked them,” said an official, who was part of the faculty team that resigned. “The matter was reported to the high-ups and the airing of grievances started to build up. We were already worried and desperate when members of other facilities started joining together and we reached a consensus and took the unanimous decision.”

The en masse resignation by the faculty members of the Shaheed Benazir Bhutto Medical College came as a surprise to the provincial administration as well as leaders of the Pakistan People’s Party, which claims Lyari to be its stronghold of decades.

A key member of the Sindh cabinet and senior PPP leader even saw the situation as “portrayed more strongly than the reality on the ground” and said the provincial administration was now trying to sort out the matter.

“I don’t rule out any incident or threat to any faculty member of the college. But one can’t declare any isolated incident to be a trend. We have, however, contacted the college administration and every possible security measure is being taken in line with their demand,” Sindh information minister Sharjeel Inam Memon told Dawn.

The security complaint from the college’s faculty also surprised the police authorities, who denied having received “any formal request” from them before they resigned.

“For the last two months, I have received no complaint, nor heard of any such things as are being reported today,” said SSP Naeem Sheikh. “We are bound to make security arrangements in this regard but we need to receive a complaint. And in this case, unfortunately, we were not approached by anyone.”Dawn.

Scanners to be installed in major cities soon: Malik

ISLAMABAD : Scanners will be installed in major cities to ensure maximum security to the citizens. This was said by Interior Minister Rehman Malik Tuesday in an interview with a private TV channel.

The fixed scanners will be installed at rush locations, after which manual checking of vehicles will not be required, he said.

The mobile and fixed scanners will be imported from China , and the first shipment will reach soon.

In the recent past, about 62 terrorists and their facilitators have been nabbed on check posts; out of which 10 were suicide bombers.

Karachi:The Pakistan Medical Association (PMA), Sindh, has strongly condemned the attitude and role of the vice-chancellors (VCs) of public and private medical universities in the promotion and establishment of night medical colleges in the country.

In a statement issued here on Wednesday, PMA Sindh President Dr Samrina Hashmi referred to a letter from the prime minister’s secretariat regarding the endorsement of the idea by all VCs and principals of medical colleges in Sindh on the grounds that by doing so medical colleges and universities would be able to make money which could be utilised for the development of medical colleges.

“The PMA strongly believes that medical education is a serious matter as doctors deal with life and death of people. All these VCs and principals of medical colleges have no idea about the consequences of this kind of an experiment,” Dr Hashmi said, adding that nowhere in the world were night medical colleges allowed or functioning.

Only in Pakistan did members of medical faculty support the idea for their vested interest and money, she charged and added that all medical universities and colleges were receiving billions of rupees in grant and they did not need this kind of money from the students of night medical colleges to run their institutes.

The PMA Sindh demanded that all VCs, principals, and the faculty members of medical colleges should condemn this proposal of night medical colleges in the interest of medical education and medical profession.

The association also warned the parents of medical students against admissions of their children to any night medical colleges as such colleges would be blacklisted at the GMC and ECFMG as per their rules and regulations.

The PMA also demanded that the PMDC should not play games and stop supporting the establishment of night medical colleges immediately. “We demand of the government to act rapidly and reconstitute the PMDC to make it competent and transparent for the better running of medical institutes in Pakistan,” Dr Hashmi said.The news.

Speaking as chief guest at a one-day symposium on ‘Curriculum Development’ organised by the Academic Section of the university at the Institute of Physical Medicine and Rehabilitation (IPM&R), KMU Vice-Chancellor Prof Dr Mohammad Hafizullah said currently the medical and dental institutions were following the annual system of examination in which the assessment is done at the end of the year.

He said most students start serious study near the end of the year and especially during the preparation leave. “To overcome this problem, most of the institutions other than medical have changed their system into semester system,” he said.

“Specific time is allotted to each course which is finished in time and there is continuous evaluation. This keeps both the faculty and students busy throughout the year,” he added.The VC said the KMU was committed to replacing the 20thcentury medical curriculum with that of the 21st century. The main feature of these reforms is adaptation of integration of the medical curriculum, he added.

Dr Hafizullah hoped that all the principals and faculties of the public and private medical colleges would support the proposed plan of KMU. Prof Dr Sultan Mehmood, principal, Khyber Medical College (KMC), informed the participants about the work done by him and his committee for introducing the semester system. The tech giant’s figures were better than expected and were helped by impressive sales of the iPhone and iPad over the Christmas period.

Apple says it sold over seven million iPads, about a million more than predicted, and also shifted more than 16 million iPhones in the first quarter which ended on December 25 2010.

The $6bn (£3.76bn) profit figure, or $6.43 per share, compared with $3.4bn (£2.13bn), or $3.67 per share, for the same period a year earlier.

Analysts had forecasted $5.41 per share for the latest quarter.

The figures were announced after regular trading ended, which saw the company’s share price close down 2.3% at $340.65 on the Nasdaq.

But the shares then rose 1.9% to $347.57 in extended trading after the release of the results.

The company is firing on all cylinders. The iPad continues to be a game changer.

Chief executive Mr Jobs’ decision to temporarily leave the firm comes two years after he took a six-month break during which he underwent a liver transplant.

In his latest statement, the 55-year-old did not say how long he expected to be away or provide any details about his latest health issues.

Chief operating officer Tim Cook will look after the firm’s day-to-day operations in his absence.

Colin Gillis, analyst at BGC Partners, said the latest Apple profits were “crazy good”.

He added: “The company is firing on all cylinders. The 7.3 million iPads, that is a great number, that is about one million more than people are looking for.”

“The iPad continues to be a game changer.”

On Mr Jobs’ health, Mr Gillis said: “Steve is an important part of what Apple is, everyone wants to know does the company fall apart? No.

“More data is always better than less (on health issues). But nonetheless they disclosed what they are going to disclose consistent with patterns.”

In an email to Apple employees, Mr Jobs said: “At my request, the board of directors has granted me a medical leave of absence so I can focus on my health.”

“I will continue as CEO and be involved in major strategic decisions for the company,” the e-mail read.

Recently, while I was working an overnight shift in the emergency department, two paramedics wheeled an elderly woman into the busy ER. She was clearly very ill: her eyes were sunken and her mouth was parched, she was slumped over and unable to do much more than moan. The paramedics told me that her family members, who had stayed home (not uncommon!), wanted to make sure we knew that she wasn’t usually “like this” and that she had recently been hospitalized at a different facility where many tests and “other stuff” had been done. Unfortunately, all her records were locked up at the other hospital’s medical-record room, which was closed in the middle of the night.

We had to start from scratch. We ordered a CAT scan of her brain to look for stroke, put a catheter in her bladder and gave her a chest X-ray to look for infection, and applied a rectal exam to look for bleeding. We may have ended up doing all of this anyway, but having more information about her recent hospitalization would clearly have allowed us to be more efficient and directed in her care. My colleague, another doctor, turned to me and said, “I cannot wait until HIT [industry shorthand for 'health-information technology'] makes this problem goes away.” (See the top 10 medical breakthroughs of 2010.)

The “problem,” of course, is that medical errors and excess costs increase when health information isn’t portable or easily accessible. The conventional wisdom is that electronic medical records, electronic prescriptions and electronic order-entry systems save costs and lives. Since 2009, the federal government has invested over $20 billion into improving HIT. And this month, the federal government will start doling out dollars to doctors’ offices and hospitals to encourage them to adopt electronic health records. On its face, this makes absolute sense — who, after all, would argue that more information isn’t essential to improved care at lower cost? During the last presidential campaign, both Senators John McCain and Barack Obama called for HIT enhancements as key to fixing health care. (See 10 players in health care reform.)

But an overzealous push toward HIT can also lead to unintended consequences. In Pittsburgh, a study showed that the rollout of an electronic system for doctors’ orders was associated with increased odds of infant deaths in an intensive-care unit. And a study conducted in Philadelphia demonstrated that computerized physician-order-entry systems facilitated medication-prescribing errors. What was going on? In Pittsburgh, medications were given too frequently because the computer used standardized dosing times to order medication (as opposed to using the time of the first dose to calculate time to the next dose). In the Philadelphia study, many of the problems arose from what are known as human-machine interface flaws. For example, doctors would sometimes assume that a display of standard doses were suggested doses specific to the patient being treated at that moment (not the same thing!). Or doctors sometimes picked the wrong medication or dose because they didn’t know that all of their options did not fit on a single computer screen and that they could click through to a second screen for more choices.

Remember the giant whiteboard used to keep track of patients and their room numbers on the TV show ER? The whiteboard is a great example of a low-tech device that can easily be customized to maximize patient care. A colleague of mine who lectures widely on patient safety shows a picture of an ER whiteboard where nurses put teddy-bear magnets next to the names of children in order to distinguish them from adult patients. Similarly, social scientists have observed how doctors and nurses gather around the whiteboard to discuss patients and their care in an effective manner. But with efforts to maximize patient privacy and go high tech, the whiteboard has largely been replaced by computerized electronic tracking systems which often reside on small monitors and frequently have no way to display information as efficiently and elegantly as a teddy-bear magnet. Of course, some of these problems will diminish as HIT systems themselves become better designed and more flexible (where I work, the computerized tracking system is customizable and has a big screen display so that doctors and nurses can easily gather around it). And implementation problems (when people are asked to do things differently than the way they have always done it, they mess it up at first) will get better as everyone gets used to using HIT. But there are some insidious factors that come with electronic health information that can be difficult to identify and measure, especially when the human element is removed from health care by technology. Here is an example: not that long ago, if I ordered an X-ray on a patient, I had to walk into the radiology-reading area to look at it. Often there would be a radiologist sitting there in the dark room; we would talk about the patient in a way that would often lead to closer examination of one part of the X-ray. Sometimes this conversation would lead to a cooperative reconsideration of the findings. But now, unless I have a specific question or concern, I don’t have to go back to the radiology reading room — HIT allows me to look at both the X-ray and the radiologist report almost instantly on my desktop computer in the ER. Sure, it is efficient and helps me see more patients and spend more time on other emergency tasks. But the result, I worry, is that I may be more likely to miss something important. Experts in the field of medical informatics attribute this error to the loss of feedback. In other words, communication should be more than just transferring information — it needs to be about getting people to act in the right way. In my X-ray case, HIT leads to standardized and reliable information exchange but unintentionally takes away the conversation, and the opportunity that conversation presents to improve patient care. So should we scale back our efforts to expand and improve HIT? Absolutely not. My elderly patient who arrived by ambulance (who, it turned out, had a colon and blood-stream infection) and the many patients like her, need to have their medical information made available in a comprehensive, reliable and efficient way. But we also need to be aware that electronic health information is not a panacea. The federal government has issued strict criteria for electronic health records called “meaningful use” which will seek to expand access to information and minimize disparities in care. But explicit plans to minimize unintended consequences of health information technology appear to be limited. HIT can change the way we deliver health care in ways that may be both hard to monitor and sometimes worse for our patients. And it is essential to keep an eye out for these influences.

Confirming that the worst law and order situation the city periodically witnessing over the past one year was keeping students away from their educational institutions, Sindh Professors and Lecturers Association (SPLA) secretary Prof Iftikhar Azmi said that attendance at public sector colleges located in the inner areas of the city had decreased by more than 50 per cent and even less than that in the colleges located in the suburbs – specially those falling under the jurisdiction of Orangi, Malir, Lyari, Landhi, Korangi and Shah Faisal towns.

“In fact, principals of the colleges are compelled to issue unlawful certificates of 75 per cent mandatory attendance to the students of their respective colleges,” he said.

He said that the problem of shrinking number of academic days for the first-year students of public sector colleges whose process of admissions under the centralised admission policy was delayed by months had become a more complex issue this year mainly because of the poor law and order situation prevailing in the metropolis for the past four to five years.

Describing the poor law and order situation as alarming, he said that parents often prefer not to send their children to their educational institutions whenever riots erupted in the city following targeted killings and, as such, attendance at most of the colleges, particularly at those located in Orangi, Malir, Lyari, Landhi, Korangi, had become quite thin.

Giving an example of the DJ Government Science College located in the heart of the city, he said that the attendance even at this college on average remained about 50 per cent so that one could imagine what would be the case in other colleges of the metropolis.

When he was asked if the recent Sindh education department decision of increasing the timings of colleges by one-and-half hour would help students to complete their course before their examinations, Prof Azmi said that it would not help much because science students already used to stay at their colleges up to 2.30pm or 3pm doing their practical courses.

“How can students, specially those belonging to first-year classes, complete their courses in just 130 days when there is an acute shortage of teachers,” he said, adding that there was a shortage of around 3,500 teachers in public sector colleges across Sindh and of them, more than 2,000 teachers were required in government colleges of the metropolis alone.

Prof Azmi, who teaches at the DJ Science College, said that even at his college where over 2,220 students studied, there were only nine associate professors against 18 sanctioned posts, besides there was a shortage of five assistant professors and 24 lecturers.

“Isn’t it a joke that there are only two teachers of English for over 2,220 students,” he remarked.

Endorsing the views of Prof Azmi, SPLA president Prof Ather Hussain Mirza said that on the one hand there was an acute shortage of teachers at government colleges across Sindh and, on the other, their science laboratories were also ill-equipped. Dawn

‘Lyari Medical College to meet PMDC criteria’
Karachi: The Pakistan Medical and Dental Council (PMDC) supports establishment of new medical colleges in the country, including the proposed Shaheed Benazir Bhutto Medical College in the Lyari area of Karachi to meet a shortage of doctors in the country.

PMDC President Prof Sibtul Hasnain Syed said this to journalists at a conference on ‘Building Bridges for Medical Education Reforms” organised by the Dow University of Health Sciences (DUHS) on Saturday.

The professor said the envisaged medical college in the downtrodden area of Lyari would meet the PMDC criteria that include the minimum 60 percent marks for candidates aspiring to seek admission to any medical college in the country.

Similarly, he said, that the college was also on its way to hire services of faculty in basic medical sciences as well as clinical sciences.

The college was already annexed with the Lyari General Hospital manned by senior medical professionals, he noted.

Answering a question, the PMDC president said that the country was exposed to a shortage of medical teachers in basic medical sciences, but different medical universities were in process of producing MPhils and PhDs in those subjects. “The country is self-sufficient in teachers of clinical sciences,”.

SSUET students bag positions in declamation contest
Karachi: Students of Sir Syed University of Engineering and Technology (SSUET) bagged position in the Allama Iqbal Shield declamation contest organized by Higher Education Commission.

An announcement of the institution here on Sunday said that the event was organized to raise awareness about the benefits of education.

In the first spell of English competition held at Sir Syed University of Engineering and Technology, Fatima Saima Ahmed, Umar Ali and Muhammad Umar Khan secured top three positions respectively while Muhammad Umar Khan, Muhammad Athar Hassan and Aariz Mahmood clinched the first, second and third positions.

Second spell of the debate was held at NED University in which teams from all over Sindh participated.

Closure of schools in kutcha areas criticised
Khairpur: Speakers at a discussion here have criticised the closure of schools in kutcha areas and described it as part of a conspiracy by feudal lords to deprive children of education.

They stressed the need for launching a campaign to press the government to reopen the schools, mostly set up during the British era.

The discussion was organised in Bhambho village in the kutcha area of Keti Mir Mohammad by the Young Boys Association, Bhitai Social Welfare Association and Bhandar Sangat to create awareness among villagers about the importance of education. The theme of the discussion was ‘Peace if people’s right’.

Left-wing activist Punhal Sario said that banditry was a conspiracy hatched by feudals and police to keep people of Sindh illiterate. When the government could hold talks with terrorists it could also negotiate with bandits to bring them back to society as responsible citizens, he said.

He said that feudal lords were using the fear of dacoits to keep people of kutcha and rural areas away from education. He said that the situation was forcing peace-loving people to leave the area. The situation in kutcha areas was similar to that in Karachi where killings and kidnappings had become the order of the day, he said.

Khairpur Civil Society chairman Mir Munawar Talpur said that education was the basic right of people and it made them aware of their rights and problems.

Khadim Hussain Mirani, Dr. Ghulam Rasool Ghumro, Comrade Hussain Bux Narejo, Mumtaz Rid, Shabbir Bhutto, Ramzan Memon, Aurangzeb Ghumro and Sultan Ghumro said that Autaqs had been a culture of kutcha areas and provided a forum to settle issues.

They stressed the need for restoring the forum and said that till 70s primary education was
common in kutcha areas which was later raised to the middle level.

They said that teachers used to educate children of fishermen in schools set up on boats in the Indus. They called upon the government to take measures to revive that culture in collaboration with social organisations. Later, a musical programme was held in which Abdul Ghafoor Soomro and others performed till early in the morning. UHS Vice-Chancellor Prof Husain Mubashar Malik said the answer key had been released for the candidates, enabling them to calculate their score on the carbon copy of the response form with the help of the answer key.

“The UHS has uploaded the answer key on its website to facilitate the candidates appeared in entrance test for 2010-11 to get correct results,” Prof Malik said.

Around 32,000 candidates took the entrance test for admission to medical and dental colleges of Punjab on Tuesday.

About the A-level candidates’ claims that some questions were out of course, the UHS vice-chancellor said that release of answer key to entrance test had dispelled the impression that the examination process had flaws.

Giving reference of a decision given by the Lahore High Court on Dec 3, 2009, Prof Malik said the court had dismissed the petition filed by A-level students in which they had claimed that the entrance test for the year 2009-10 was out of course. He said the court had examined the record and statements produced in the court from both sides and finally gave decision in the favour of the UHS.

The vice-chancellor also referred to an excerpt from the verdict which said: “As regards the questions on which the objection has been raised that those are out of the course or syllabus, it may be held that the respondent-university has provided us the complete details of the relevant questions and the reference books/material from which the test has been prescribed. From such books we do not find that the grievance of the petitioners is well founded and genuine.”
